Where can I find my bank routing and account numbers?

You can usually find these numbers in your online banking account, on a bank statement, or along the bottom of a paper check. Do not use the number printed on a debit card. Contact your bank if you are unsure which numbers to provide.

What should I do if the request form is not working?
